Joint replacement bundles go national in 2028. Start building now

Joint replacement bundles go national in 2028. Start building now

CMS says its joint replacement bundle saved Medicare $180 million without hurting quality, and that result is the case for CJR-X, which makes the model mandatory for most hospitals in 2028.

The evaluation

CMS released the final evaluation of the Comprehensive Care for Joint Replacement (CJR) Model on Sept. 24, reporting $180 million in Medicare savings while quality held steady for lower extremity joint replacements. The original model was mandatory in 34 metropolitan areas for more than eight years starting in 2016.

What CJR-X requires

The FY 2027 inpatient payment rule finalized CJR-X, a national version of the bundle that starts Jan. 1, 2028, three months later than CMS first proposed.

  • Who is in: Hospitals paid under the inpatient and outpatient prospective payment systems. The 716 hospitals already in the TEAM model join when TEAM ends after 2030, and Maryland hospitals are excluded.
  • How it pays: Episode spending is compared with a risk-adjusted target price each performance year. Depending on spending and a composite quality score, a hospital may earn an additional payment or owe Medicare money.
  • Risk adjustment: Target prices use 29 risk adjusters, the same set as TEAM, compared with three under CJR.
  • Protections: Rural and safety-net hospitals get a 5% stop-loss limit. Hospitals with fewer than 31 eligible procedures a year must participate and report data but are not subject to payment reconciliation.

Why 2028 is closer than it looks

Bundles reward what happens after discharge as much as in the operating room. Hospitals that did well in CJR built preferred post-acute networks, cut unnecessary inpatient rehabilitation and skilled nursing stays, standardized implants and set expectations with surgeons and patients before surgery. Those changes take a year or more to build, and they depend on surgeon alignment that many hospitals do not have today, especially as joint procedures continue moving to ambulatory surgery centers. Hospitals already in TEAM can borrow from that work, while those new to bundles should start with a baseline of their own episode spending and outcomes.

Executive takeaway: Pull your current joint replacement episode costs and post-acute patterns now, name a physician and administrative lead for CJR-X, and use 2027 to build the care pathway before the risk arrives.
